Your first visit

A little preparation helps us make the most of your time. Please bring:

  • A photo ID and your insurance card
  • Any relevant imaging (X-rays, MRI) or reports you already have
  • A list of current medications
  • Details about your injury or symptoms and what makes them better or worse
